Middletown Youth Football has decided to let the National Center for Safety Initiatives (NCSI) provide us with the background
checks this year. Attached is a copy of the instructions for filling out the “Background Screening Self Registration”. Follow the
link below to the website and click on “Background Screening Self Registration”. You will need to use registration number: 29951823.
